Welcome to Commerce Primary Care

At Commerce Primary Care you will be greeted with friendly smiles and positive energy, as we pride ourselves in providing an uplifting patient care experience at every visit.

Commerce Primary Care opened in 2004 when husband and wife team, Dr. Samuel Jonnalagadda, and Dr. Alka Jain started their own practice. Since then the practice has enjoyed the success of comfortable growth while still maintaining a close personal connection with each patient. Each and every member of the CPC practice takes pride in the fact that their patients are treated like family.

Our physician’s, clinical providers, and staff are highly trained in providing exceptional care for a broad range of concerns. We believe that the clinical visit does not end when you exit the building, but rather is the initiation of an ongoing conversation and relationship. Our skilled staff believes strongly in prompt same day follow up for all questions and concerns, as they arise.

 

We are specialists of adult medicine and have extensive training in all aspects of general and acute healthcare. We are proud to be designated a Patient Centered Medical Home (PCMH). We focus on prevention, diagnosis, and treatment of a wide range of medical conditions, including diabetes, high blood pressure, heart disease, lung disease, arthritic disorders, high cholesterol, weight management, depression, and geriatrics.

We promote “healthy living” by placing an emphasis on teaching, coaching, and prevention. But, if our patients do need more extensive medical care, we provide continuity by having our own doctors follow our patients at the hospital. The practice is affiliated with DMC-Huron Valley Hospital.

Our office provides a variety of medical services including: physical exams, preventive care for chronic disease, same-day sick visits and telemedicine visits. We also provide on-site services to our patients including blood draws, immunizations and injections.

Patient Forms

Insurances Accepted

If your insurance plan is an HMO, please call the member services number on the back of your insurance card. Make sure you have selected Dr. Samuel Jonnalagadda or Dr. Alka Jain as your PCP before your appointment.

We DO NOT accept electronic refills from local pharmacies.

We ask that our patients contact our office directly at 248-363-7500 (prompt 3) for future refills.

What’s New and Exciting at CPC?

We have a new financial policy at Commerce Primary Care. Please see the appointment tab located at the top of this page for more information. There is also a PDF available with our financial policy available.

You can request to have access to the portal through our website or the next time you come into the office we can send you a request through your personal email.

Our portal is a secure form of communication that gives our patients 24-hour access to their personal health information. Patients can view health information such as recent doctor visits, medications, allergies, lab results, diagnostic results, etc. Other great features are the ability to request an appointment, exchange secure e-mail with their health care team, request prescription refills, update demographic information, download, and complete forms.

Patients can now check in for their appointment through their portal

Due to insurance carriers changing their “preferred labs” and now that there are so many different insurance plans available to patients, it is the patient’s responsibility to find out which lab is considered in-network or “preferred” to ensure their labs are covered. Some insurance carriers will only allow their members to use certain labs such as Quest Labs, Lab Corp, etc. and our office is not responsible or capable of knowing which plans prefer which labs. Our office NOW uses Quest Labs. There is also a Quest patient service center located in the Commerce Walmart that has Saturday hours. If a patient has labs done at our office and they are not covered by their insurance due to lab coverage, the patient may be responsible for those costs, so our office highly recommends checking with your insurance before your appointment!

We have a patient reminder system in place. Patients will receive a reminder of their appointment real-time and another reminder two days prior if made in advance. Patients will also be notified once their prescriptions have been sent to the pharmacy. Next time you’re in the office, please make sure we have your preferences updated so we know how you would like to be contacted!

Our office has a 2 business day refill policy. When calling in to the office, please follow the phone prompts to the RX/Test Result line (prompt 3), please leave a detailed message with your name, DOB, medication, dosing and preferred pharmacy.

If you are completely out of a medication, please follow the phone prompts to reach the Front Desk (prompt 2). All controlled substances require a 3 month controlled substance appointment. Maintenance medications still require a yearly physical or 3-6 month planned visit for chronic conditions for refills to be given.
