Dr. Samuel Jonnalagadda

Dr. Samuel Jonnalagadda, M.D.

Dr. Sam is a Board Certified Physician in Internal Medicine. His passion and dedication to people led him down an unlikely and surprising path as Dr. Sam chose to open his own practice directly after training. His strong work ethic and love for medicine supported the undercurrent of a rapidly growing private practice. His calm and caring nature helped to nurture each individual patient relationship. A unique quality that Dr. Sam possesses is his ability to treat each patient respectfully and compassionately, regardless of the situation.

Dr. Sam completed his undergraduate degree through University of Michigan in 1993 and graduated from Saba University School of Medicine in 1999. He completed his residency in Internal Medicine in Grand Rapids at Spectrum Health – MSU program. He and his wife Dr. Jain started the practice in 2004. They are both devoted to creating a warm, patient-centered atmosphere that focuses on each patient’s needs. Dr. Sam rounds daily at Huron Valley Sinai Hospital where he provides direct patient care for his personal office patients who have been admitted into the hospital. Patients love to see Dr. Sam on rounds, as he usually can make them laugh and forget about their illness for a while. It is important for Dr. Sam to keep up with continuity of care for his patients, especially the ones with complex medical issues. He makes sure to follow up with his patients at all transitions of care (office, hospital, nursing home, urgent care). He makes himself available 24/7 to his patients via pager. When Dr. Sam is not working, he enjoys music, sports and spending time at home with his wife and kids.

 

Dr. Alka Jain

Dr. Alka Jain, M.D.

Dr. Jain is a Board Certified Internist who takes pride in having a strong work ethic and meticulous attention to detail. Dedication to patient care is exemplified by her strong interpersonal skills, solid background of medical knowledge, and ability to effectively relate to patient’s needs. These qualities are the backbone for the success of the private practice, Commerce Primary Care, started in 2004 by herself and her husband, Dr. Sam Jonnalagadda. They are as devoted to their family as they are to their patients. She met her husband in Ann Arbor where they completed their undergraduate degrees at the University of Michigan. She went on to obtain her MD from American University of the Caribbean. Her and her husband completed their Internal Medicine Residency in Grand Rapids in the Spectrum Health – MSU program.

Since starting her career in medicine, Dr. Jain has started a very successful private practice and currently serves as the practice manager. She really enjoys the “business” development aspect of medicine. She has also been a medical director for a large Sub-acute Rehab and National Hospice and has worked as a hospitalist. She has served on several hospital and nursing facility committees in relation to Transitional Care, and recently was named to the Board of a Pioneer ACO. As she served as Medical Director for a Sub-acute Rehab, she was blessed to have been able to serve as a physician in so many different aspects of the medical spectrum. With the knowledge she has gathered, she hopes to promote excellence in patient care while developing her passion for transitional care. She is particularly passionate about our seniors, and hopes to help them age with grace.

 

Rachal Graff

Rachal Graff, P.A.-C.

Rachal Graff earned her Masters of Physician Assistant studies at Wayne State University in May 2016. Prior to this, she graduated from Michigan State University’s Honors College where she received her Bachelor of Science degree in Physiology. She loves all aspects of medicine and particularly enjoys doing a wide range of procedures. Outside of work, Rachal has been enjoying life as a new mom to her beautiful daughter and spending time with her husband and dogs. Rachal worked as a medical assistant for us prior to going to school and has now come full circle in rejoining our team at Commerce Primary Care!

 

 

 

Jessica Graham

Jessica Graham, N.P.-C

Jessica is a board certified Family Nurse Practitioner. She graduated from Boston College with a Master of Science in Nursing in 2010, and has been a valuable part of our medical team ever since. During her time with Commerce Primary Care, she has practiced internal medicine in a variety of settings, including our primary care office and various sub-acute rehabilitation facilities (working alongside Dr. Jain). Jessica has a special interest in holistic medical treatment throughout the lifespan, management of chronic diseases, and preventive medicine. She has a particular passion for helping her patients implement and sustain healthy lifestyles for the long-run. Outside of work, she enjoys spending time with her family, vegetable gardening, cooking, biking, traveling, hiking, kayaking and camping. Jessica has been with CPC since December 2010.

Lana Elia

Lana Elia graduated with high honors from South University as a certified Family Nurse Practitioner and a Master of Science degree. Prior to that she obtained her Bachelor of Science in Health Sciences and a Bachelor of Nursing from Oakland University. Lana has a diverse background in health care where she previously worked as a surgical and home health care nurse. She is committed to making a difference in patient’s lives and providing the best quality of care. Her passion for practicing medicine stems from personal experiences that drive her to always put patients first. Her compassion for helping and healing is evident upon an initial acquaintance. Outside of work she enjoys spending time with family and friends, traveling, and reading. She loves being a part of the Commerce Primary Care family and committing herself to the welfare of her patients.

Rachel went to Grand Valley State University where she received her Bachelor’s Degree in Elementary Education in 2010. She is in charge of the building maintenance, drafting patient letters, health insurance incentive initiatives, employee matters and other important aspects of maintaining the practice to ensure patient care. She enjoys reading, being outside and spending time with her family. Rachel has been with CPC since June 2011.

Cheryl has an Associate’s Degree in Business from Macomb Community College and has also earned a certificate in billing, coding and medical office administration from Oakland Community College. Outside of work she enjoys spending time with family, scrapbooking and is a huge fan of the Detroit Tigers and Red Wings. Cheryl has been with CPC since March 2011.

Jaclyn Edwards

Jaclyn Edwards

Jaclyn is part of our front office staff and is a certified Medical Assistant.  She handles our patient check in and check out area. She obtained her certification through Ross Medical Education Center. Her background includes Ob/Gyn and internal medicine at local offices. She handles inputting new patients into the system, updating any necessary forms and verifying insurances. She provides excellent patient care from the moment a patient steps foot in the door and will assist that patient in whatever they need to make sure they are satisfied. Jaclyn enjoys spending time with her son, being with friends and working in the gym. Jaclyn has been with CPC since June 2014.

Natalie Pardington

Natalie is part of our front office staff and attended Baker College of Auburn Hills and recently completed her CNA certification through Warren Health Academy. She also obtained certification in 2006 as a Health Unit Coordinator. Natalie has worked in Otolaryngology as an administrative assistant and medical assistant. She handles patient calls, scanning of important documents, sorting through medical records and ensuring patient satisfaction with every interaction she has with our patients. Outside of work she enjoys spending time on the boat, skiing, snowboarding, working in the gym and spending time with her family. Natalie has been with CPC since December 2014.

Suzette Peterson

Suzette is a part of our front office staff and has many years of experience in a local Ob/Gyn office.  She assists with many important aspects in the office; sorts through our daily faxes, scans important documents into patient charts, handles phone calls and takes on anything else that comes her way. She enjoys spending time with her family and friends outside of work. Suzette has been with CPC since September 2015.

Brandy Patyna, MA

Brandy is one of the front desk receptionists for the practice. Her roles include answering telephones, scheduling patient appointments; verify insurance and taking messages using the practice electronic system. Brandy is one of the first people that you will see when you arrive at the practice. She is always available to answer your questions and prides herself on providing the highest level of customer service possible. Brandy is also helping our medical assistants with patient care and procedures.

Katie Wiese, CMA

Katie is a certified medical assistant at Commerce Primary Care. You will find her meeting patients at the lobby door to escort them to the exam room while talking with them about their day. She checks vital signs, medication lists, and discusses the reason for the visit as she enters the information into the practice’s electronic medical records. She assists the physicians with procedures, responds to messages from patients, and addresses clinical questions that come into the practice.

Andrea is a medical assistant for the practice. In her role, she is responsible for ensuring that the back office duties such as updating medication lists, checking vital signs, and discussing the reason for the visit are obtained and then entering the information into the patient’s electronic medical record prior to the physician seeing the patient. She enjoys talking with patients and hearing about how they have been since their last visit or getting acquainted with patients who are new to the practice.

Berenice Segovia-Lopez, MA

Berenice is a medical assistant for the practice. Her role includes checking and recording vital signs, discussing with patients the reason for their visit, medical problems, reviewing current medication and future needs for prescription refills and referrals are all part of her duties. She also responds to telephone messages from patients and supports the physicians in making sure all of your medical information is properly recorded in your electronic medical record.
