Your First Visit at Commerce Primary Care
Commerce Primary Care is currently accepting new patients. We are not currently accepting new patients with Medicaid or self-pay. To schedule an appointment, please call 248-363-7500. Same day appointments are available. Each patient will be asked to fill out a few basic forms. Our staff would appreciate it if you come in 20 minutes before your scheduled appointment to fill out these forms. We can also email them to you upon your request. On your first visit be sure to bring your health insurance card, driver’s license, co-payment if applicable and a list of current medications. A link to the new patient forms is attached below. You can print these forms and bring them with you to your appointment.
