Visiting us for the first time
Visiting us for the first time? Read this section to prepare for your appointment.
We welcome all new patients. It is our goal to make your first visit as convenient and comfortable as possible. To help achieve this goal, established patients should arrive at least 10 minutes early to update any necessary information. New patients should arrive at least 20 minutes early for paperwork.
Remember to bring
- Your insurance card(s).
- A list of current prescriptions and/or over-the-counter medications you are taking, including dose and frequency.
- A list of any herbal or nutritional supplements you take, these can affect your body and its reaction to other medications we may prescribe for you.
- Pertinent information about your medical and surgical history. Any recent x-rays, if necessary, and your previous medical records (if possible) sent to our office prior to your visit. This aids the physician with your treatment. We will also request medical records from previous physician(s) or hospitals once the new patient packet is complete with the release of information page.

Confirmation & Cancellation:
A phone call is received from our appointment reminder system, reminding you of your upcoming appointment. If unable to keep your scheduled appointment, call our office 24-hours in advance. Your phone call is critical in helping us provide continuous care to all of our valued patients. If you fail to give us notice of your cancellation, the following policy is enforced:
First missed appointment
No charge
Second missed appointment
$25.00 charge
$25.00 charge and possible dismissal from Commerce Primary Care
Third missed appointment: $25.00 charge and possible dismissal from Commerce Primary Care
