Insurances Accepted
As of May 1, 2025 we will no longer be accepting HMO. We hope with this advance notice current patients are able to join an alternate insurance option when it comes time for your insurance to renew.
We DO NOT participate with any Select Plans, Focus Network, or Metro Detroit HMO. Please check your insurance cards to confirm that these are not mentioned as your carrier.
If your insurance plan is an HMO, please call the member services number on the back of your insurance card.
Make sure you have selected Dr. Samuel Jonnalagadda or Dr. Alka Jain as your PCP before your appointment.
While we may be credentialed with a particular plan,
we may not be accepting new patients under that plan at this time.
If you do not see your insurance listed below, or have questions if we are still accepting new patients under your plan,
please call the office to confirm or check with your insurance company to see if our office participates.
